After you sign up, pay the fee, and go get fingerprinted, submit your proof of fingerprinting here.

Click on the "Fingerprinting Overview" link and follow the instructions.
To find a fingerprinting site, go back to the link and click on "Print Location and hours" and then click on #3 on the map to find a site nearby. Make sure the site you choose is currently open.
